In the wake of the disaster, a wave of troubling testimonies from former employees suggests this accident may have been foreseeable. Documents reviewed by NPR and CBS News reveal that pilots and safety officers had spent years warning management about systemic failures. Karl Seuring, a former union president for the airline’s pilots, claims he was fired in 2022 specifically for voicing concerns over safety standards. Similarly, former safety director Don Helmig allegedly warned ownership back in 2021 that safety protocols were being treated as mere formalities. These whistleblowers allege they faced pressure regarding mandatory rest periods and felt discouraged from reporting hazards—claims that 21 Air has since denied, insisting they follow all federal regulations.

Beyond the immediate failings of the carrier, the crash has placed Amazon’s broader logistics strategy under a microscope. Rather than running its own fleet entirely, Amazon relies on several third party contractors like 21 Air to move its goods. This model is now facing intense scrutiny over how much oversight the retail giant actually exercises regarding the internal cultures of its partners. NTSB Chair Jennifer Homendy confirmed that investigators are looking closely at the contractual obligations and safety provisions shared between Amazon and its contractors to determine if gaps in accountability contributed to the tragedy.

Amazon has pushed back against these criticisms, arguing that partnering with certified air carriers is standard industry practice and stating that it takes all safety allegations seriously. However, regulators are now questioning whether current safeguards for outsourced cargo operations are sufficient when red flags are raised long before a fatal event occurs. As investigators pore over two hours of cockpit audio and dozens of hours of flight data from the 1994-built aircraft, the focus remains split between the technical errors made during those final seconds in Miami and a potential pattern of negligence that preceded them.
