Lilly raises diabetes weight-loss bar with amylin-Zepbound combo

Eli Lilly is pushing the boundaries of metabolic medicine with a new combination therapy designed specifically for people battling both obesity and type 2 diabetes. The experimental drug, known as eloraTZP, combines the popular weight loss medication Zepbound with an amylin receptor agonist called eloralintide. In recent study results presented at the European Association for the Study of Diabetes meeting in Milan, patients using the cocktail experienced an average weight loss of 23.3 percent. Those on the highest dosage saw even more dramatic results, shedding an average of 54.1 pounds over a period of 48 weeks while also seeing improvements in their A1C levels.

What makes these findings particularly significant is that treating weight loss in patients who also have type 2 diabetes is notoriously difficult, often resulting in less impressive numbers than those seen in patients without the condition. Analysts from Citi noted that these results are comparable to what Zepbound achieves in non diabetic populations and comfortably exceeded their own expectations. By targeting three different hormones—GIP, GLP-1, and amylin—the drug mimics complex biological signals that regulate appetite, slow digestion, and block excess sugar production in the liver.

However, this potent approach comes with some hurdles regarding patient comfort. The trial revealed that gastrointestinal side effects were more common when the two drugs were combined than when either was used alone. These issues led nearly 27 percent of participants to stop treatment entirely. Experts suggest that these dropouts might be caused by starting both medications simultaneously rather than gradually increasing the dose, meaning Lilly will likely refine how the drug is introduced to patients during upcoming trials to improve tolerance.

Despite these teething problems, Lilly appears confident enough to move forward with phase 3 trials planned for late 2026. The pharmaceutical giant is currently locked in a high stakes arms race with competitors like Novo Nordisk and Zealand Pharma to dominate the next generation of obesity treatments. While other companies are exploring similar amylin combinations, Lilly’s latest data suggests it may hold a competitive edge in addressing the intertwined complexities of blood sugar management and chronic weight loss.
